Please download and print an application for the Title IV-D Child Support Program and additional questionnaires listed below. Complete the forms and return them to the Child Support Division office at your earliest convenience. Your application will not be processed if it is not complete. Because the Application, Notice and Waiver and questionnaires require your signature, you need to fill out a print copy.

Enclose with your application and questionnaires:

• Money order for $25 payable to the Indiana Department of Child Services (do not send cash or personal checks)
• A copy of your child(ren)'s birth certificate (Health Department copies only)
• A copy of the child support, divorce decree and/or modification to the order/decree
